Our mission is made possible by providing home care, social services, personal care, Adult and Social Day programs, as well as health promotions. Services are offered in all five boroughs of New York City, as well as Nassau and Suffolk Counties.

Our Mission

EXTENDED MLTC is committed to maintaining the highest ethical and moral standards. Our employees and providers focus on providing compassionate care while emphasizing the physical, emotional, and social needs of our members. We are able to accomplish this by partnering with quality health care professionals to promote and maintain optimal health and independence within the home
