Military Appreciation Month is a time to reflect not only on the service of our nation’s Veterans, but on our collective responsibility to support them when they return home. 

At Community Hope, that responsibility is at the core of everything we do. For decades, we have been dedicated to helping individuals and families overcome mental illness, homelessness, and poverty—with a deep and unwavering commitment to Veterans. We believe that the words homeless and Veteran should never go together and our work is grounded in providing not just services, but stability, dignity, and hope.

What Are Common Veteran Challenges When Transitioning Out of Service?

The Veterans we serve often face complex and interconnected challenges. Many are navigating housing instability or homelessness, while also managing mental health conditions, including PTSD. Others encounter barriers to employment or experience a profound sense of isolation after transitioning out of military life. These challenges rarely exist in isolation, which is why our approach focuses on whole-person care—addressing both the clinical and non-clinical needs that are essential to long-term stability and well-being.

How is Community Hope Addressing Some of These Challenges?

To meet these needs, Community Hope has built a comprehensive continuum of care designed to support Veterans at every stage of their journey. Our Veterans housing programs provide safe, stable environments that serve as a foundation for rebuilding lives. Through supportive services, including case management, employment assistance, and skill building groups, we help vVeterans regain independence. Our rapid rehousing and homelessness prevention case management programs offer compassionate, individualized care, and our Case Managers ensure that each Veteran is connected to the right resources at the right time. Our SSG Fox Suicide Prevention Grant Program team is able to provide critical support, resource navigation, education, lethal means safety, and much more so that Veterans and their families don’t have to go it alone. 

As the needs of Veterans have evolved, so too have our programs—expanding to provide more integrated and responsive support.

How Do Cross-Sector Partnerships Improve Veteran Outcomes?

Central to this work is the understanding that no single organization can do it alone. Partnership is not just a strategy for Community Hope—it is a necessity. By working alongside the U.S. Department of Veterans Affairs, The NJ Department of Veterans Affairs, The NJ Department of Community Affairs, local housing authorities, healthcare providers, and corporate and philanthropic partners, we are able to extend our reach and strengthen our impact.

Utilizing Unite Us to receive closed-loop referrals also allows us to expand our reach, especially nationally. Unite Us also allows us to have a secure platform to communicate with referring entities and Veterans who refer themselves for services. Through Unite Us, we can also collaborate with referring entities in order to reach Veterans who may be hard to reach. These collaborations create a more seamless system of care, helping to ensure that Veterans do not fall through the cracks and can access the services they need more quickly and effectively.

Cross-sector collaboration also allows us to think beyond immediate needs and toward long-term solutions. 

By leveraging the strengths of our partners, we are better equipped to address the root causes of homelessness and mental health challenges among Veterans. This network of support enables us to deliver more coordinated, holistic care—ultimately leading to better outcomes for those we serve.

What’s Next for Advancing Veteran Care?

Looking ahead, we are inspired by the opportunity to expand our impact even further. We are committed to reaching more veterans, deepening our partnerships, and continuing to innovate in how we deliver care. Santa Fe, New Mexico – April 29, 2026 – Founded in 2017, Santa Fe CONNECT represents a bold, unified vision between Santa Fe County and the City of Santa Fe. Since 2019, County and City staff have collaborated with one another to establish a care coordination and navigation network that connects individuals to health care and public services through trained navigators embedded across clinics, public agencies, and community-based organizations.

CONNECT started off with 8 programs and 13 navigators who worked together to break down silos, expand community partnerships, and encourage collaboration across the county. CONNECT has transformed the care coordination network in Santa Fe into a powerhouse network of 79 organizations and 300+ navigators who are collaborating, leveraging resources, and supporting individuals via a closed loop referral platform.

A new case study of CONNECT reveals how a government-led, technology-powered approach to care coordination is delivering measurable economic and health impact for the community. Powered by Unite Us, the nation’s trusted technology partner for integrating health and community-based care, the initiative has generated more than $33 million in estimated healthcare savings and economic value for households since 2019, while building a more accountable system of care for residents.

New Hampshire Care Connections strengthens coordination between healthcare providers, community-based organizations, and state partners to better support the health and well-being of Granite Staters.

The Dartmouth Health Upper Valley Community Partner Pilot was established with DHHS and Unite Us, the state’s technology partner for integrating health and community-based care. In collaboration with the Granite United Way, the Pilot is creating a regional network of health care and social service providers working together to improve how individuals and families access essential health and social services by creating a coordinated, secure referral system.

“Partnerships like the Dartmouth Health Upper Valley Community Partner Pilot are critical programs for us at Dartmouth Health in our ongoing work to improve health outcomes in the communities we serve,” said Joanne M. The AVRC serves as the hub for the new statewide network, powered by Unite Us, the nation’s trusted technology partner for integrating health and community-based care, bringing together organizations spanning workforce development, healthcare, housing, education, benefits navigation, and behavioral health to securely send and receive referrals, track progress from start to finish, and ensure Veterans do not fall through the cracks.

“We are so excited to welcome the community into our space today as we continue building a strong, connected support network across Alabama,” said Alex Karagas, Executive Director of the Alabama Veterans Resource Center. “This Open House is about growing relationships and bringing more organizations into a shared effort to serve those who have served us. An expansion of the state’s Connect Oregon network, this public-facing directory enables individuals, community health workers (CHWs), and care providers to quickly identify and share local community-based services. 

Since its launch in 2020, Connect Oregon, powered by Unite Us in partnership with OHLC, 211info, Oregon CCOs, and health systems has supported coordination between health care providers, government agencies, coordinated care organizations (CCOs), and community-based organizations (CBOs) across the state. The network facilitates care coordination, closed-loop referrals, and CBO reimbursements, serving nearly 80,000 Oregonians to date through more than 150,000 referrals addressing drivers of health such as housing, food, utilities, and transportation. Connect Oregon also supports several workflows for Oregon’s 1115 Medicaid Waiver, facilitating more than $30M in HRSN reimbursements to service providers.

The new PRD extends the reach of Connect Oregon directly to the public, reinforcing Oregon’s commitment to a “no wrong door” approach to care. Individuals can now self-navigate available resources, while people working in the field, such as CHWs, care coordinators, and outreach staff, can easily share verified public-facing resource links via text, email, or print.

The report reflects how Unite Us partners, including health systems, government agencies, health plans, and community-based organizations (CBOs), met a rapidly evolving policy and funding environment in 2025 by using technology and data to better predict needs, connect people to services, close the loop on outcomes, and sustain community care.

In 2025, Unite Us partners faced increasing pressures to prove impact, show cost savings, and sustain programs amid shifting policy and funding systems. To support evoloving needs, Unite Us continued to advance its platform across three areas: 

• Expanding its Predictive Analytics capabilities used by communities to get upstream with proactive interventions
• Enhancing its Closed-Loop Referral System with more robust screenings, integrations, and resource recommendations, 
• Strengthening its Payments tools to speed up community reimbursements.

The report also spotlights partner milestones across the country: from reducing avoidable emergency department utilization, to reclaiming thousands of staff hours in county governments, to supporting chronic disease outcomes through food and nutrition interventions. These examples reinforce a broader industry shift toward greater transparency, accountability, and scalability for community based care.
