We applied that time savings to the total number cases referred by Intermountain Health organizations since the adoption of Unite Us, and estimated that <\/span><b>2,155 hours of administrative efficiency were gained since 2019 <\/b><span style=\"font-weight: 400;\">across all referring Intermountain Health organizations.<\/span>\r\n\r\n<img class=\"aligncenter size-full wp-image-11752\" src=\"https:\/\/uniteus.com\/wp-content\/uploads\/2026\/02\/workflow-fig2-IH.png\" alt=\"\" width=\"1585\" height=\"572\" \/>"},{"acf_fc_layout":"wysiwyg","_acfe_flexible_layout_title":null,"_acfe_flexible_toggle":null,"id":"","wysiwyg":"&nbsp;\r\n\r\n<hr \/>\r\n\r\n&nbsp;\r\n<h3>Healthcare Impact<\/h3>\r\n<span style=\"font-weight: 400;\">We estimate that by October 2025, Intermountain Health has saved up to <\/span><b>$7M<\/b><span style=\"font-weight: 400;\"> in total cost of care for <\/span><a href=\"https:\/\/uniteus.com\/blog\/transform-medicaid-with-a-self-sufficiency-model\/\"><span style=\"font-weight: 400;\">Medicaid<\/span><\/a><span style=\"font-weight: 400;\"> patients.\u00b2 \u00b3<\/span><span style=\"font-weight: 400;\"> If every Intermountain Health Medicaid member was served by Unite Us, savings could have exceeded <\/span><b>$182.4M<\/b><span style=\"font-weight: 400;\"> during the same time period. <\/span>\r\n\r\n<span style=\"color: #ffffff;\">.<\/span>\r\n<h2><img class=\"aligncenter wp-image-11943 size-full\" src=\"https:\/\/uniteus.com\/wp-content\/uploads\/2026\/02\/graph-intermountain-v5.png\" alt=\"\" width=\"673\" height=\"311\" \/><\/h2>"},{"acf_fc_layout":"wysiwyg","_acfe_flexible_layout_title":null,"_acfe_flexible_toggle":null,"id":"","wysiwyg":"&nbsp;\r\n\r\n<hr \/>\r\n\r\n&nbsp;\r\n<h3>Service Utilization and Resolved Needs<\/h3>\r\n<img class=\"alignright wp-image-11800\" src=\"https:\/\/uniteus.com\/wp-content\/uploads\/2026\/02\/top-services-IH3.png\" alt=\"\" width=\"424\" height=\"260\" \/><span style=\"font-weight: 400;\">Figure 5 highlights the most common types of services accessed by Intermountain Health patients through Unite Us, illustrating where <\/span><a href=\"https:\/\/uniteus.com\/products\/closed-loop-referral-system\/\"><span style=\"font-weight: 400;\">coordinated referrals<\/span><\/a><span style=\"font-weight: 400;\"> most frequently resulted in resolved needs. <\/span><a href=\"https:\/\/uniteus.com\/case-study\/healthy-eating-pilot-hba1c-ny\/\"><b>Food assistance<\/b><\/a><b> represented the largest share of resolved needs (25%)<\/b><span style=\"font-weight: 400;\">, reflecting sustained demand for access to services such as food pantries, emergency food support, SNAP\/WIC enrollment and prepared meals. <\/span><b>Physical health-related services accounted for 17% of resolved needs<\/b><span style=\"font-weight: 400;\">. Notably; dental care accounted for 39% of all resolved physical health needs, highlighting oral health as a key area of unmet needs addressed through community-based referrals. <\/span><b>Housing and shelter services comprised 14%<\/b><span style=\"font-weight: 400;\"> of resolved needs, followed by <\/span><b>mental and behavioral health (9%) <\/b><span style=\"font-weight: 400;\">and <\/span><b>individual and family support services (8%).<\/b>"},{"acf_fc_layout":"wysiwyg","_acfe_flexible_layout_title":null,"_acfe_flexible_toggle":null,"id":"","wysiwyg":"&nbsp;\r\n\r\n<hr \/>\r\n\r\n&nbsp;\r\n<h3>Network Reach<\/h3>\r\n<span style=\"font-weight: 400;\">The Unite Utah network expansion has been driven largely by Intermountain Health, now connecting residents to <b>more than<\/b><\/span><b>\u00a01,000 unique programs <\/b><span style=\"font-weight: 400;\">from nearly <\/span><b>500 on-platform, engaged partner organizations<\/b><span style=\"font-weight: 400;\">. As a direct result of this increased collaboration, patients who have been referred into the network have seen indirect benefits from Intermountain Health\u2019s work, with an additional <\/span><b>$1.8M in accessible income among 594 patients.<\/b>\u2074\r\n\r\n<img class=\"alignright wp-image-11768\" src=\"https:\/\/uniteus.com\/wp-content\/uploads\/2026\/02\/active-programs-IH2.png\" alt=\"\" width=\"350\" height=\"226\" \/>\r\n\r\n<span style=\"font-weight: 400;\">Key features of the network also include the Utah Coordination Center (UCC) operated by the United Way of Salt Lake. Since 2020, the UCC has served <\/span><b>1,337 clients for 2,396 needs.<\/b><span style=\"font-weight: 400;\"> When finding best-fit partners on the platform for client referrals, community partners have shown a 62% resolution rate in meeting their needs. In instances where a best-fit partner is not available, UCC utilizes the off-platform resources functionality of the platform. These values represent additional money available to meet household needs due to services rendered (e.g. $50 in food from a food pantry 2 x per month).<\/em><\/div>\r\n<div style=\"margin: 0 0 4px 0;\"><em>\u00b2 Savings was calculated by applying the $85 PMPM savings found through the North Carolina Health Opportunity Pilot adjusted for 2% inflation year-over-year to the estimated number of Intermountain Health medicaid beneficiaries. The low end assumes that 11.3% of Intermountain Health patients served by Unite Us were Medicaid beneficiaries based on Utah\u2019s projected Medicaid population from the Kaiser Family Foundation, while the high end assumes all clients served were Medicaid beneficiaries. <\/em><\/div>\r\n<div style=\"margin: 0 0 4px 0;\"><em>\u00b3 Like the North Carolina HOP findings, the explicit factors driving these estimated cost savings are unknown but likely stem from decreased healthcare utilization (e.g. fewer readmissions)\r\n<\/em><\/div>\r\n<div style=\"margin: 0 0 4px 0;\"><em>\u2074 Additional accessible income was calculated by estimating the average household benefit for each service subtype\u2014using publicly available regional cost equivalents\u2014and multiplying that value by the number of resolved cases in each
