Seattle, WA \u2014\u00a0<\/b>HealthierHere\u00a0today launched the Connect2 Impact Alliance, a new partnership with Ready Computing and Unite Us designed to\u00a0streamline\u00a0how community organizations, health\u00a0and social\u00a0care providers, and government agencies collaborate\u00a0to improve community well-being. Built with\u00a0and for\u00a0local communities and already\u00a0used\u00a0around Washington State, Connect2\u00a0Technology\u00a0enables partners to securely share information, coordinate referrals, and support whole-person care.\u00a0<\/span>
\n<\/span><\/p>\n

The\u00a0new\u00a0alliance unites three complementary partners:\u00a0HealthierHere, which co-designed\u00a0Connect2\u00a0and leads its governance and long-term stewardship; Ready Computing, which provides Channels360,\u00a0the interoperable technology enabling secure, consent-based information sharing across existing systems; and Unite Us, which powers the statewide closed-loop referral network that connects more than 600 community-based organizations, healthcare providers, and public agencies.\u00a0<\/span><\/p>\n

Unlike solutions that require organizations to\u00a0start from scratch\u00a0or overhaul existing technology, Connect2 was designed to integrate with current systems, or provide a case management system\u00a0for those looking for one.\u00a0The\u00a0technology\u00a0enables consent-based information sharing, improves visibility\u00a0of\u00a0care across providers, and helps ensure people can\u00a0connect to\u00a0the services they need through\u00a0closed-loop referrals. Connect2\u2019s governance and consent model\u00a0is shaped by\u00a0diverse stakeholders,\u00a0ensuring\u00a0that privacy, trust, and local priorities\u00a0are central\u00a0to how information is shared.\u00a0<\/span><\/p>\n
